Recognition Chart

PURPOSE: Record the recognition you provide team members throughout the week.

Time: 1 Hour

STEP 1: Record your team members’ names on the Recognition Chart.


STEP 2: Each time you provide recognition to a team member, place a check mark next to the person’s name.

Note: See if you can place a check mark next to each team member’s name at least once a week.


STEP 3: At the end of each month, tally the number of times you provided recognition for each team member.

When reviewing the summary, think about the following:

  1. Are there team members who received more recognition than others? Why?
  2. Do you see any patterns regarding who receives the most recognition? (e.g., high performers vs. low performers)
  3. What team members need more recognition?
